i was actually able to reproduce this on one of my devices and i found out why the album art never changes in this case. When it goes to look up the album art for an album, it cannot find the album in the database so it just skips over all the code to change the album art.
I changed it around to behave like one would expect.. it searches the sdcard for the art

About first song playing twice in a random order playlist, I keep having that: think you can try and reproduce that making a new database scan (with discard old database option), go in file browser, long tap on a folder and play. I have first song playing two times, no matter if I just let it play or move to next song. EDIT: of course I tried and reproduce the "two-times" bug but I couldn't do that when I wanted to... Anyway, random play still starts with first song in playlist, and that's for sure and happens all the time I fire up a new playlist. Did you also check those FLACs on Dropbox? Would like to get them offline ASAP.

i did download the flacs but havent looked at them yet, so you can take it offline
I do have a minor bug that i noticed that might be related to the song playing twice. When playing a folder of folders from the browser.. the next track doesnt get set (if you have next track visible on now playing it will just show as blank) so something obviously isnt getting set correctly until the next track is played. What might be happening is just simple luck that the same track plays twice in a row. The first track plays, but isnt flagged as "already played", so when it randomly picks the next song.. that first track is a valid selection
I will try to do what you recommended to reproduce it
Ah so playing the first track happens when you load a playlist? Okay i can see that happening... i probably never put anything in to pick a random first song for playlists (it should randomly pick a first track when selecting folders / albums etc)
I personally rarely use playlist files so its understandable how i missed that

k i can reproduce the always playing first song with folders... i do know why.. playing folders is handled diff from most things since its not known immediately how many songs are going to be added to the playlist (it has to traverse through the folder/subfolders first).. so the logic is different and i guess i never added the part where it picks a random position and plays that

I havent thought of a good way around it yet. This latest patch was pushed out early because playback didnt work at all in 4.1, so I had to release a fix for that.
