I have a Galaxy Nexus running Android 4.0.2 ICS and I'm having the following issues:
Every setting from "Enable Headset/Remote Control" and bellow in the Headset area doesn't work.
The music player doesn't catch or react to the headset button being pressed. The result is that the stock music player starts/stops when ever I press the button on the headset (I have tried every possible settings combination).
Also the setting where you select to use the volume controls to go between tracks doesn't work (have tried short, long and double presses)
I will report back if I find some more bugs/issues

yea there are issues with teh remote on ICS currently. My only ICS device is a tablet and it my current remote doestn work at all on it (in any app)
the volume button to change tracks is only supposed to work when the screen is on (which is not very useful right) does it not even work with the screen on?

Volume button works while screen is on like you say it should, but not in lockscreen.

k.. yea that is all that feature does currently.. i've been researching for a LONG time how to listen for volume presses when the screen is off but have been unable to find out how. Most posts say it cannot be done (without actually running a modified android rom).
I know it is possible. whether the solution is a hack or not.. because there is an app called tactile player that is able to pick up the volume key presses

I have seen one or two players that has it as well so there is a way to do it, but the button on the headset should be the top priority.

poweramp supposedly can do it.. but when trying on my phone it does not do anything.. most likely an unsupported api is being used that isnt guaranteed to be on every device.

okay was able to find a widget on the market called media buttons that acts like a remote control.. and can confirm in ICS it will only control google music instead of gmmp.. i have a fairly good idea why

well i coded up a work around.. its ugly so i really hope google fixes the issue.. i'll try to get the patch out tomorrow

just pushed 1.1.15 to the market. Give it a try and let me know if your headset works. I wasnt able to test it with an actual headset, but I used some media button widgets that send out the same intent as headsets do.

For a quick hack as I like to call it when I make quick work arounds for some issues while programming it works quite good.
Some minor issues that I have (nothing that needs to be fixed over night though) I need to turn on the screen for the media button to work if the player has been in pause for 5-10 minutes.
I also noticed another minor thing and that is that if I for example press the media button 2 or 3 times to go to next/previous track it will pause and start the playback 2-3 times.
If you manage to keep up the pace you have now with pushing out fixes and updates then it won't take long before your player becomes one of the best ones on the marked.
And it's fun to see a fellow developer that is so eager to improve their code.

i wonder if the device goes into some sort of sleep? I do have some settings that will keep a wake lock to prevent sleeping.. but I know that is not the greatest for battery life. One good way to test that would to see if the same thing happens with the native music player

Do you have menu -> preferences -> general -> single button mode enabled?
That allows the multiple presses to do different actions. You can actually customize the action for single/double/triple clicks and single/double/triple click and holds
