I'm using GMMP v2.2.1 on a Moto X Play (XT1562), running the Stock ROM on Android v6.0.1 (Android security patch level = "1 January 2017", Kernel version = "3.10.49-g7089fb5")
I was fiddling with the settings in the "Effects" screen, when suddenly the volume from GMMP jumped very high, all by itself, while changing the BassBoost setting. I would guess this 'Runaway' volume was at 100% volume, but I have no way to confirm that since there was nothing indicated in the UI.
I reduced the volume by the hardware volume button, which showed the volume going to 0, but this had no impact on the GMMP volume, which continued playing at runaway level. The sound was uncomfortably loud; I had to pull out the earphones from my ears in a few seconds.
After I pulled out the earphones from the 3.5mm jack, I then tried to play GMMP via the Speakers. Here too GMMP played at full volume. Re-attaching the ear-phones again had GMMP playing at runaway level, even though the hardware volume was at 0.
Only after explicitly selecting "Exit" from the GMMP menu, and re-launching, GMMP obeyed the hardware volume setting.
I have hit this bug twice. In both cases, I recall modifying the "Virtualizer" setting first, and then when enabling/disabling the "Bass Boost" setting, the GMMP volume ran-away, disobeying hardware volume controls. Note that I had not modified the Equalizer option, which was left disabled.
I've noted the below setting in GMMP preferences, that seemed relevant to me.
Not Enabled/Disabled settings:
Use GMMP Equalizer
Use External DSP
Enable Crossfade
Enabled settings:
Enable ReplayGain
Prefer Album Gain
Enable Custom AudioEngine
Bass Boost Strength 40%

When I disabled the custom AudioEngine, I was not able to hit this bug again so far, even after fiddling around with "Virtualizer" and the "Bass Boost" settings in the "Effects" screen. So this bug seems to be originating from the custom AudioEngine.
